چکیده

INTRODUCTION Since George Beatson and A. Schinzinger produced remissions of advanced breast cancer by performing bilateral oophorectomy in premenopausal patients more than 100 years ago, the effect of antihormonal therapy on breast cancer has been known. Initial endocrine therapy included oophorectomy, hypophysectomy, and adrenalectomy. The latter two were abandoned when tamoxifen citrate was approved by the US Food and Drug Administration for use in advanced breast cancer in 1978. Selective estrogen receptormodulators (SERMs), such as tamoxifen, antagonize estrogen receptor (ER) function by binding competitively to it. Since 1978, more than 40,000 breast cancer patients, most of them postmenopausal, have been treated with tamoxifen, either in the context of clinical research studies or in clinical practice.

Long-term efficacy and safety of letrozole for the adjuvant treatment of early breast cancer in postmenopausal women: a review

Aromatase inhibitors (AIs) are becoming more widely used than tamoxifen as adjuvant hormonal therapy for postmenopausal women (PMW) with early breast cancer (EBC). It is clear that these drugs offer important efficacy benefits over tamoxifen and differ from tamoxifen in their safety profile. The accepted strategies for adjuvant AI therapy include initial adjuvant treatment following surgery, sw...

Selecting adjuvant endocrine therapy for breast cancer.

This year alone, more than 215,000 women in the United States will be diagnosed with, and over 40,000 will die from, invasive breast cancer. Recently, mortality from female breast cancer has declined despite an increase in its incidence. This decline corresponds with improved screening for prompt tumor detection, and advances in the treatment of early disease. Of these, endocrine therapy has pl...

Effect of adjuvant chemotherapy in postmenopausal patients with invasive ductal versus lobular breast cancer.

BACKGROUND On the basis of the lack of response of invasive lobular breast cancer to neoadjuvant chemotherapy, we questioned the effectiveness of adjuvant chemotherapy in relation to histology. PATIENTS AND METHODS Women with primary nonmetastatic invasive ductal or (mixed type) lobular breast cancer, aged 50-70 years, diagnosed between 1995 and 2008, were selected from the Netherlands Cancer...

Aromatase inhibitors in breast cancer therapy.

Recent advances have been made in the hormonal treatment of breast cancer with the advent of third-generation aromatase inhibitors (anastrozole, letrozole, and exemestane). These newer agents have substantial antitumor activity and appear to be as effective as tamoxifen, with fewer adverse effects. Recent reports indicate that anastrozole is more effective than tamoxifen as adjuvant endocrine t...

The role of bisphosphonates in the adjuvant setting for breast cancer.

Bone health is a critical issue in the management of women with breast cancer. Many women who develop breast cancer are postmenopausal, which already predisposes them to osteoporosis. Systemic treatments for breast cancer, including chemotherapy and endocrine therapy, decrease circulating levels of estrogen in both pre- and postmenopausal women, further accelerating the natural process of bone ...
